9/4/2023 EnchantaVaughn Approved Behavior: It was noticed by QMHP, client had a tired look on her face but switched moods once session started. Client’s energy increased once she got comfortable in the home, initiating a conversation with QMHP. Behavior: Client did not have to be directed on sharing or waiting her turn, and QMHP praised her for her excellent behaviors going into the second half of the session. Client was receptive to the idea of practicing breathing technique and meditation strategies. Client was cooperative and appeared to be learning a lot from today’s session. Intervention: QMHP inquired about morning experiences during initial in-home contact with client. QMHP asked how client was feeling and if she needed anything. QMHP will review problem solving and give feedback regarding ADHD and its causes and effect to well-being and mental performance. QMHP encouraged client to be receptive to all feedback and to take in what is being said rather than how it’s being said. QMHP will teach client meditation skills to decrease any mental or emotional tension. QMHP assisted client in developing more communication skills. Intervention: QMHP assisted client in normalizing the changes that are occurring in the home and outside the home. QMHP reflected with client on her choices, reviewing both good and bad experiences. QMHP processed with client, "how can you add more value to your life?”. QMHP modeled pro-social behaviors for client to strengthen her understanding of what to do in the program both in person and virtually, as well as, at home and out in the community. QMHP encouraged client to have a good evening upon ending the session. Response: Client transitions smoothly and safely to session area, displaying appropriate behaviors. Client was seen waiting for instruction. Client understands that problem solving starts with thinking before acting. Client demonstrates problem solving by staying calm and using her senses. Client acknowledges her improvements and vows to stay consistent with her therapy. Client have been doing a better job controlling impulsive and hyperactive behaviors, due to keeping her hands to herself and staying in designated area. Response: Client exhibits appropriate behavior in the home, as observed during the role play activity. Client displays good participation and displayed good interaction with her family, acknowledging their love and support for her session time. Client understands that sessions are a privilege and she have to show good work ethic and follow along with the interventions, in order to maintain his ISP objectives. Client continues to make progress in the home, as reported and observed. Client did an excellent job turning her day around and reaching her goals.
